What is the tax form I need for the $17 k I'm taxed on for domestic partner insurance?

Domestic Partner Insurance I am taxed on every week. For example $387.
For a total over $17k I am taxed on as income for what the medical insurance is worth. What tax for do I file on this?

The $17K will already be included in box 1 of your W-2, as imputed income. You just report your W-2, as usual. There is no special handling.
