How do I get forms from Turbotax to apply for tax installment plan?

You can apply for an IRS Installment Payment Plan right in TurboTax.  When you go through the FILE section, you'll be asked how you want to pay your Balance Due, and you can select that method.

 

The IRS will mail you verification.  The interest rate and payment amounts are very reasonable.

You would have to see if payment plans are available with your state.  List of Department of Revenue by state  List of state DOR's.  @WenHN
