Question

What should I do with "Vermont Income TaxPayment Voucher 2018 Voucher IN-116"? Should I send it out together with my check to pay the tax? Or simply just the Voucher?

Yes, send the voucher with the check for taxes due. The purpose of the voucher is to give your Social Security number to the state government since they almost certainly use your SSN to know where to credit the payment.
