Question

When filling out the 1099g form .. it’s asking for payers name and address is that yourself? Or the department that sent you the 1099. And also part two question.. on my 1099g form on box one it

On my 1099G form on box one it has an amount for unemployment compensation but on the bottom part in box number 1 it has a zero amount which one would I put on the form that asks you to put the amount

The payer's name and the address is for the department that sent you the form.

You should report the amount in box 1 on the top part and ignore the 0 at the bottom.
