Question

When filling out the wages and income part there is a box asking "Received From?". What is entered here? Name of firm, number ...what?

    It would be the payer of the income source.  So, it generally would be the bank or investment firm that paid you the interest/dividends etc.  For example, if you earned $100 in interest from savings account with TD Bank - you would put "TD Bank" in the "Received From" entry.
