Question

When the first name has a space, there is confusion of how it's treated

My brother's first name has a space. This had caused quite a bit of issues. In his driver license, it seems they treated the name after the space as middle name. However, in prior years' tax return, his tax preparer was able to enter the first name with a space and left the middle initial empty.  We want to correct the situation to match his driver license in this year's tax return by just splitting the first name into first and middle in the 1040. We were wondering is there an additional IRS form that we need to file to indicate the first and middle name is different from prior year?

For the tax return, his name needs to match his social security card, not his drivers license.  I would file it the way that his prior preparer did, since you know that version was accepted by the e-file system (assuming that you are filing electronically).

On the U.S. tax return there is only room for a single middle initial after the first name.

There is no IRS form to indicate a name change, as any name change would need to be made instead with the Social Security Administration.
