Where do I enter long term care policy numbers and insurance company name?

You don't have to enter the Insurance company name and policy number on the federal return, but you include the amount paid in Medical expenses:

  1. Select Federal from the left side menu.
  2. Select Deductions and Credits toward the upper left. 
  3. Expand the list and scroll down to Medical Expenses.
  4. Click Continue until you get to the screen titled, How much did you spend on insurance premiums?
  5. Look for Gross long-term care premiums (there will two boxes on a joint return) enter your amount and Continue.

If your state requires more information, you will prompted for it when you go through the state interview
