Thanks for the info, but my initial question is where do I insert that form into the turbo tax program. In previous years it was easy to add the form. This year I can’t find the link to use it.

Sorry but I don't know anything about Indiana state tax returns. I have never seen anyone who answers questions on this site answer one about Indiana. You might want to post the question again and start over with a new post since people won't answer a question that already has answers.
