Where do I find a form R-6410

I am being asked for a credit recovery from form R-6410. I have never received this form before and have no applied for any tax credit that I am aware of. I’m not sure why I’m being asked

The LA form is used to give you credit. Perhaps you worked in another state or one of the other categories suits you? Here is a link to the form so you can see if it applies to you or if you need to delete it from your return.
