TurboTax will create the form for you.  You will make the entries in the "Job (W2)" topic from the information you have (usually last pay stub).  You will select the "did not receive W2" box in the follow on pages to generate the Form 4852. 

 

Be sure you have exhausted all avenues before going this route.
