As per the Canada Revenue Agency (CRA) I am unable to find a box 45 on the T4 information slip. Employee-paid premiums for private health services plans would be included in box 85.

You will need to discuss this with your employer. As there is no amount in box 85 it seems you may be able to pay for your own dental coverage through your employer: you are eligible to access but not necessarily paid for by the employer. This is new this year for the employer and was not updated on the CRA's employee information. You may also want to enquire directly from the Canada Revenue Agency at 1-800-959-8281.
