How do I know if I qualify for the Ontario Energy and Property Tax Credit?

I am separated, but my home as of November 2018. I am under 64 years of age.
If I do qualify, what do I enter in the "Number of Months" field, months I have lived on this property (26x12), or how many months I have independently owned the home (5x12)?

You would enter the number of months you lived in the home in 2023, so 12 if you were there all year.
