There are many descritions and many dates ???

I searched in IRS website and have not seen this form " capital asset sales worksheet" ??

The information that TurboTax is asking about should be on your Form 1099-B. If you go to the link, scroll down to the second page.

The capital assets sales worksheet is a worksheet TurboTax generates with the Form 1099-B information you entered. It is not an IRS form.

The information you enter from your 1099-B form(s) is then reported on Form 8949 Sales and Other Dispositions of Capital Assets.
It is then transferred to Schedule D (Form 1040).
