If a 1099-DIV statement from an institution has a *"NOTE: This  information is not reported to IRS", Do I need to report this on my tax return??

    If it is not reported to the IRS  you should still report it on your tax return if it is income, dividends that you received.  All income is required to be reported even if it is not reported to the IRS.
