Question

Cost basis questions-investment was received as a gift decades ago but we do not have dates to provide. Should we estimate or leave blank?

Put in your best estimate instead of leaving it blank.  The primary reason would be to determine if it is a long-term or short-term holding, so as long as it is over one year it will be long-term.
