Do I need to bother with Schedule D? I only made $11 in dividends and Investment Total Short Term and Grand Total are Proceeds: $108.40, Cost Basis: $104.40.

$4 profit in the forced sale of two shares. Hardly seems like it's worth paying $69 for Federal plust $39 for the State of Michigan.

Yes, if you are filing a return, the IRS requires you to include all income from all sources.
