Do I report quarterly ESPP purchases? Shares were not sold.

Confused because there is an option to report ISO purchases if you did not receive a 3921.

You don't report simple "purchases" of ESPP shares.  The reporting requirement comes when you sell.

Companies are supposed to prepare a Form 3921 for purchases of stock via an ISO and a Form 3922 for purchases of stock via an ESPP.  These forms help you determine your basis when you do sell the stock.

IF you buy stock via an ISO and DO NOT SELL all that stock before the end of the year then you do have to enter the Form 3921 to see if you've run afoul of the ATM.
