entering 1099-B form

I'm filling out my 1099-B form I had sold some stock on the form it is asking for the date acquired and the cost or other basis. On my 1099-B form it list N/A for both of those. What do I put for the date acquired and cost or other basis?

    Do you know when you acquired the stock and how?

    • To start with, did you purchase it?
    • If so, when?
    • Determine the date the best that you can.
    • Stocks generally list there stock value on their website, or their are entire websites that store historical data. Market Summary 

    It is your responsibility to maintain records regarding stock transactions.

    If you leave the date and cost basis as is, you will be taxed on 100% of the distribution.
