How do I list my dividends without a 1099-DIV?

I only made $5.39 via Merrill Edge investments last year, so I didn't get a 1099-DIV. However, before I moved my Coca-Cola stock to Merrill halfway through the year, I had made around $30 in dividends that were deposited into my checking account.

 

I want to accurately list all of this, but I have no forms to upload, and I see no way to list any of it without doing so via 1099 in TurboTax. Is there a section I can just list the total amount and call it a day?

Report the amount received as if you had received a 1099-Div.

Right, but I don't know who the financial institution is. I guess it'd be Merrill Edge for the second half of the year, but how do I list the dividends I received from my certificate stocks prior to moving them to Merrill?

Since the dividends were not listed on a form 1099-DIV, you can make one entry for all of them. Enter them as if you were entering a form 1099-DIV. If you don't have a broker name, enter "NA" for not available.
