What kind of form should I have showing the sale of the stock?

If you sold stock through a broker, you should have received Form 1099-B, which will show the date sold, and proceeds, and if available, the purchase date and cost basis. If you did not receive a Form 1099-B, then you should contact your broker and request one. If you online access to your account, you should be able to download a copy.
