My one of my tax reports has a section of 'Mutual Fund and UIT Supplemental Information'. My state is DC. Details of this section include the following information:

FOREIGN SOURCE INCOME PERCENTAGES

How and where do I enter this information while filing my taxes?

Just enter the amounts shown on the page(s) that show a 1099 item such as 1099-INT, 1099-DIV, or 1099-B.  If on the 1099-DIV there is a listing for "Foreign Tax Paid", identify the items on the detail forms that foreign tax was withheld from and keep a running total of Total Dividends and Qualifying Dividends.  More and more firms are now listing foreign items separately for easier identification.

 

The percentages you refer to above reflect the percentage of income attributable to the items listed.  For instance, you list Fed Source Total 0.13%.  You would allocate 0.13% of that particular fund's income to Federal sources not subject to state income tax.  Unless the total is substantial, any change in tax amount at the state level would be negligible and probably not worthy of the effort to calculate.
