Stuck on Form 1040-x , cannot fill in , see attached

Hi Everyone,

 

I have received great advise this week thank you all, I am stuck on how to complete my  amended 2020 return. Can anyone please advise how to complete the form? See attached.

    Please clarify.  Cannot see attachment.
