Question

There is no form 4562 on my 2016 tax return for my dental practice. The schedule C does show the deduction but I need the breakdown. Thanks!

Did you add any new assets in 2016?  If not, then there would not be a Form 4562 included with the tax return.
Look at the Depreciation and Amortization Report for current and prior depreciation for assets that you have included and reported as an expense on Schedule C.

Basically from my own personal experience, you have to elect the option to print *E*V*E*R*Y*T*H*I*N*G* in order to get the 4562's to print. The options for "forms needed to file" or "forms to keep for your records" will not print the 4562's.

It's a good idea to  always save a paper copy of the return or at least a PDF (with & without the worksheets)  & the .taxfile of your return each year right AFTER it has been accepted by the feds & state.  Then you will always have access to your returns in the future.
