Where do I enter dividend income received without having a 1099-DIV form?

financial institution did not provide a 1099-DIV

You simply use the 1099-DIV interview entering the information "as if" you actually received one. 

 

If you look at the actual Schedule B where interest and dividends are reported

https://www.irs.gov/pub/irs-prior/f1040sb--2019.pdf

you'll see that there's nothing there that indicates whether you received a 1099-DIV or not.  It's just a listing of payers and amounts.
