Question

1099-G only has 5 boxes. Box 1: Unemployment Compensation Paid, Box 3: Federal Tax WithHeld, Box 4: State Tax WithHeld but it does not have a Box 10a, Box 10b and Box 11?

On intuit turbotax it has boxes different from mines such as Box10a - State, Box 10b - State ID number and Box 11 - State income tax withheld. Mines does have a Box 11 but it is Box 4  and Box 3 - Federal Income tax withheld. So should i just ignore Box 10a and Box 10b and just put the value for Box 11 since that's all my 1099-G has?

If you enter information in Box 11 (State Tax Withheld), you will also have to enter information in Box 10a (State) and Box 10b (State Identification Number). If your 1099-G does not have a identifying number on their form, you will have to either call the state or find their ID number on their website. Most states will publish their state identifying number for unemployment on their webpage.
