Box 1 (Gross Distribution) has an amount listed. Box 2a (Taxable amount) is 0. Box 2b is marked with an "X"  "total distribution". Do we need to report this on our income tax since we did not make any withdrawals?

Yes. You will need to include this form on your tax return. Even if it isn't taxable, it is required for informational purposes.
