1099-R has IRA/SEP/SIMPLE

My 1099-R has IRA/SEP/SIMPLE box that is checked or marked with an X, what option do I select in turbo tax when it asks: Do any of these situations apply to you? 

This is for a SIMPLE IRA within the first two years of participation only
 
This is for a SIMPLE IRA with more than two years of participation
 
This is for a SEP IRA
 
OR
None of these apply

Can you provide more detail? What kind of retirement account do you have? If you have a Traditional IRA then you select "None of these apply".
