My wife just received a 1099-R for a gross distribution she never received. What do we do?

    Contact the Payer that is on the Form 1099-R to find out what happened.

    You don't need to report income on your tax return that you didn't constructively receive, but you need to investigate as mentioned by @DoninGA . If the money was available to you and you didn't receive it for some reason, it would still be taxable if you could have secured it by the end of the year. For instance, if you received a check in the mail but didn't cash the check until the next year, it would still be taxable to you in the year you received it. If you find that the funds were not available to you then you need to request a corrected Form 1099-R from the issuer.
