If box 14 is empty, or a zero, then try leaving boxes 14, 15, 16 entirely blank.

If box 14 has a non-zero value in it, then box 15 must have "OH" in the first part of it, and a number in the second part of it  (that is, as long as you were a full year resident of OH)......and for box 16, you should copy the value of box 2a into box 16 (as long as box 2a is not blank, and not a zero or marked as being "Undetermined)........and if you made no after-tax contributions to that particular plan, you can use the box 1 value in box 16.

Yes, some states require a value be placed in box 16, even if empty on the 1099-R tha you received.
