2019 Forms for Qualified Disaster Distributions

I got the following message:

"you can't file just yet due to a recent law passed by congress. This may mean a better tax outcome for taxpayers like you who have Qualified Disaster Distributions... We'll let you know when we can help you finish your taxes"

I was impacted by Hurricane in Oct 2018, but didn't request 401K withdrawal until early 2019.

 

1. Which form would I need to file? is it Form 8915-C or Form 8915-D?

2. When do you expect these forms to be available?

3. Can/Should I pay the extra taxes and penalty now and amend my return once these forms come out?
