Question

All boxes are blank except my proceeds (box 1d). How do I fill out the rest?

    There should be other data, like a buy/sell date.  You may need to look over past documentation so you can provide information about this sale. if you don't the basis, the entire amount may be capital gains, so may need to rely on your broker or past documentation to supply the missing details.
