As an IHSS worker, Do I have to report social security tax witheld shown on W-2? and if I do where do I enter the information???

You don't include this W2 in your filing.  The income you are receiving, since you are providing the services in your home, are not taxable to you, and therefore the W2 should not be reported on your return.  

For some, the payments are subject to Social Security and Medicare withholdings.  You would want to contact IHSS for more information on this or if you qualify for a refund of those withholdings.  If you do, though, the request for the refund is claimed through IHSS; it is not claimed on a Federal (or California) tax return.

My wife is IHSS caretaker for her mother, who lives with us in California.  My wife is retired and she a got W-2 that doesn't show earnings or fed tax, but they did take medicare and social security withholding.  Is this just understood that IHSS should be refunding the medicare and social security with holdings, or is the somehow known under the tax code?

As DanielV01 mentioned above, it is not automatic.  You should contact IHSS to find out if a refund is due.
