Box 16 - State Distribution on 1099-R is blank but am being asked to fill it in. What do I do?

    Do boxes 14 and 15 have entries? If not, then box 16 should not have an entry either.

     

    But if you have entries in boxes 14 and 15, then it is likely that the entry in box 16 will equal the entry in box 1.
