Can I recharacterize a roll over from a terminated 401k plan into a Roth over to an IRA?

With the taxable amount in box 2a then that is indeed a 401(k) to Roth IRA conversion.

Unfortunately the 2018 Tax Cuts and Jobs Act eliminated the ability to reverse (recharacterize) Roth IRA conversions.

Was this a conversion from a 401(k), or a 401(k) Roth rollover?   What code is in box 7 on the 1099-R?
What is in box 2a.
Anything in box 5?
