Di I need to add a 1099-R that shows a gross distribution (I didn't receive any money) but under taxable amount it says "0.00"?

Code W means Charges/payments for purchasing qualified long-term care insurance contracts under combined arrangements.

 

You don't have to report this form 1099-R on your tax return. Just keep the form with your tax records for 3 years.
