Do I amend 2019 return if I didn't include a 2019 1099-R (gross distribution, code 4G, 2a $0) for a govt 403b I inherited in 2018 & rolled into inherited IRA in 2019?

I already filed 2019 and received refund.

There is no need to amend.  The only omission on your filed Form 1040 is the inclusion of the box 1 amount on line 4c and the word ROLLOVER next to line 4d.  The IRS should recognize that a code 4G Form 1099-R with $0 in box 2a is a nontaxable rollover anyway, and Form 5498 from the inherited IRA should show a rollover contribution of the same amount distributed from the 403(b), indicating that the rollover was completed.  Just be sure to retain account statements to further substantiate that the rollover was completed, in case the IRS ever asks about it.
