Do I attach form 1099R to my Hawaii State Tax Return when mailng it in

The form turbo tax printed it only says attach copy 2 of Form W-2. Instructions say attach copy of Form(s) W-2 and 1099-G. I'm not sure about the Form 1099R, which I have been provided by certain Payers.

Generally, only if a 1099-R has tax withheld do you have to send it in.

If there's something in box 12 of your 1099-R, send it in.

Yes. I followed the Turbo Tax directions which only said W2 or 1099-G. Received a letter from the State of HI Dept. of Taxation asking that I attach the same thing, which I did not have; so I called. They said attach the 1099-R's that show State withholding.
