Do I enter the amount that's in bracket 12 a D in the additional contributions?

A W-2, box 12, code D ??

 

Box 12 Code D on a W-2 is for 401k (or TSP) contributions.

 

A 401k is NOT an IRA.   Your W-2 box 12 Code D entry goes in on your W-2 entries...and nowhere else.

 

Additional contributions you are asked about on the Deductions&Credits page are only for those made to an outside account actually labeled as being an "IRA" .... thus, just other contributions you might have made to a traditional or Roth "IRA"....and only those labeled as being "IRA"s
