Do I have to report a W-2 that doesn't have an amount in box 1 and only has an amount in box 12?

The IRS does not allow e-fileing a zero box 1 - no nothing - W-2.

 

If box 13 (3rd party sick pay) is checked and the box 12 code is J, then the W-2 is for your information only  and can be ignored.
