Do I have to report my employers contribution to my traditional IRA if my match contribution was to a Roth?

No, you do not enter contributions to a work retirement account they are already included in box 12 of your W-2.

 

Only enter traditional IRA and Roth IRA contributions that you had outside of work in the IRA contribution section.

 

Please be aware, Form 1099-R is used for distributions and not for contributions.
