Do I need to file my 2019 1099R form if I lost the retirement rollover check and requested a stop payment and a new check in 2020?

I lost my 2019 retirement rollover check issued in 2019. I requested a new check in 2020, but I'm unsure if I need to file my 2019 1099R form.

Yes, you must report it.    You had constructive receipt of the money.   Losing the check is your problem and does not change the reporting requirement.

 

Hopefully there was a code G in box 7 (and the check was payable to your IRA and not you)  that does not have a 60 day rollover rule otherwise the IRS might not grant a waiver of the 60 day rollover rule since it was not a bank error but your own fault if the 60 period was missed, then the entire distribution might be taxable income and can no longer be rolled over.
