Do I owe ohio state taxes I withdrew from my ira that was a transfer from my employee 401k

I paid state and local tax on that money before money put in my 401k

No. If handled as a roll over on the federal return, and not taxed, it will not transfer to the Ohio return as taxable income.

When you entered the 1099-R, with the withholding in boxes 4,12 & 15, the withheld tax will be credited to you state and School District (local) return and be refunded to you.

"Local tax"? City or School district? IRA distributions are not subject to city tax.
