Question

Do I still need to have form 1099-R even if I did not took my RMD from that institution. I withdraw my RMD from another account.

    No, if you took your total required RMD from only one institution, you do not need a 1099-R from an institution that you did not receive any payments from.  The 1099-R is only issued from companies that make payments to you.
