Do you need to file form 5498-SA? Keeps asking me for infomation from that form and then doesn't accept answer.

The 5498-SA tax form is used to report contributions to a health savings account (HSA).  It includes scheduled payroll contributions one-time contributions, contributions from your employer and contributions from any other party on your behalf.  It is for informational purposes and is not required to file a tax return.  If you contributed to an HSA through payroll deduction, your HSA contributions would also be reported on your W-2.
