Question

Hello, I received a 1099-R form. Does this mean I should check the box when turbo tax asks, "nonqualified pension plan not on my W-2"?

In addition, I don't know why I received this 1099-R form I don't remember requesting any type of funds to be withdrawn. Thank you.

No, you should not necessarily check that box.  What was your Form 1099-R issued for? And what type of a plan is it from?  Is it an Employer Provided Retirement Plan? If so, it is likely not a "nonqualified pension"

What is the code in Box 7 of your Form 1099-R?  Where is the 1099-R issued from? Can you ask them why you received it?

You may have been required to take a "Required Minimum Distribution" from your pension plan if you are over age 70 1/2 or 72 depending on your birthdate.
