Hey, team! I’m positive I withdrew money from my 401k, paying a penalty for doing so when I withdrew the money. I did this early 2019. However, I went on my Fidelity account for my tax document,

Fidelity stated I wouldn’t be receiving tax documents for 2019... how do report the withdrawal ? Or do I report it, if there is no document ? I’m not sure of the exact amount to report.

suggest calling Fidelity and discuss.... that ensures you do this correctly. ask them WHY you won't be receiving a 1099-R form. Also, ask them to confirm the date of the transaction.
