Hi I have a 401k from an old job and didn't get anything on it is this something I still have to put on my taxes thank you

No, you do not enter a 401(k) from a job in which you are no longer employed.

In the event that you take a distribution (cash out), it will be designated on a 1099-R tax statement, you will claim it

You also have the option of rolling it over to another financial institution such as a bank or mutual fund company such as Fidelity or Vanguard in the form of a Rollover IRA.  If you decide to do so, make sure to have it rolled over "Trustee to Trustee" (without receiving a physical check)
