I took a $38000 distribution from my Roth IRA in 2024 and I was 61 years old. I'm being asked to list my "regular Roth IRA Contributions prior to 2024". Where would I find this?

Generally, you should have tracked your Roth IRA contributions, but you could check Form 5498 if you kept them all.

In you case, if you had the Roth IRA for 5 years then your distributions are Qualified Distributions. You Form 1099-R should have code T or Q. Therefore, you can ignore the question about the net contributions prior to 2024.
