Are you referring to box 2a of your Form 1099-R?  Please clarify.

Yes, on information for 2a. The Annuity Statement says UNKNOWN. It won't let me put in UNKNOWN.

They recommend I use the Simplified Method. Where do I put that on the form.

The Simplified Method is used to determine the taxable amount of periodic distributions. When you enter Form 1099-R, specify that you receive regular distributions, but there's nothing in box 2a, TurboTax gives you the option to use the Simplified Method to determine the taxable amount of your distribution.

Review the TurboTax article Where do I enter my CSA 1099-R? As you go through the interview, TurboTax will ask you questions about your 1099-R so you can use the Simplified Method.
