How can I report qualified rollover from trad IRA, if 1099R will not be issued?

I also rolled over 401K to the qualified annuity.  They sent me 1099R.  The company for the Trad IRA told me they would not because it was direct rollover.

The movement that you did of one traditional IRA to another was done by trustee-to-trustee transfer.  Such a trustee-to-trustee transfer of an IRA is neither a distribution nor a rollover, so it is not reportable.  On the other hand, movement of a 401(k) to an IRA can only be done by distribution and rollover, so movement of a 401(k) to an IRA always must be reported.

If there was no 1099R issued for the rollover, then you do not have a reporting requirement reporting the rollover.
